Credentialing Information

It is the Policy of EPMHMR that all Professionals employed by or who contract with EPMHMR that participate in the service delivery system undergo a credentialing process that reviews the Professional's licensure or certification, training, experience, possession of special skills, competence, and all other pertinent background information. This process is repeated at least every three years and culminates in the approval or disapproval of clinical privileges of the applicant by the EPMHMR Board of Trustees. This Policy also applies to individual non-licensed Professionals seeking certification as Qualified Mental Health Professionals (QMHP) or Qualified Mental Retardation Professionals (QMRP).

EPMHMR requires all Professionals who provide services to consumers
to be credentialed before services are delivered.

Please complete the following:
  • cover sheet to credentialing application
  • credentialing application
  • peer letter, minimum of three
  • forward completed cover sheet, application and 3 peer letters (identified peer indicated on each letter) to:
