Assertive Community Treatment (ACT)

The objective of the ACT Team is to reduce inpatient admissions and to increase the quality of life of individuals who historically have difficulty remaining in a community setting. Utilizing initiative and independent judgment, the team provides aggressive intervention with consumers, family members, and service providers. The team provides an array of services, including: medication monitoring, symptom management skills training, advocacy, coordination of services, and community based hospitalization for consumers assigned to the unit. In addition to their normal hours of operation, team members remain "on-call" to provide intervention 24 hours a day if needed.
